BUFFALO, SD – Buffalo Regional Medical Clinic will provide free sports physicals for school athletes from 8 a.m. to 5 p.m. on Tuesday, July 13 and Friday, July 30 at the Clinic, located at 209 Ramsland St., in Buffalo. The physicals will be performed by Crystal Page, PA-C, Patricia Stephenson, M.D. and Leroy Krueger, PA-C. There is no charge for the physical.
Students requiring a physical to participate in middle or high school sports are asked to bring in the completed South Dakota High School Activities Association sports physical forms. These forms must be filled out and signed by a parent or guardian, and are available at the school, the clinic, or online at www.sdhsaa.com.
